New Patient Appointments

New York Nephrology accepts new patients by referral from another doctor. If you have been referred to our practice, please contact the physician who referred you to confirm the status of your appointment.

What to Bring to Your First Appointment

No fasting is required for your first appointment. If testing is needed, you will be given a return appointment.

Please bring the following items to your first visit:

  • All current medications in their original containers

  • Insurance cards

  • Photo identification
  • Co-pay
  • Referrals, if required by your insurance
  • Copies of your medical records, if they have not already been supplied by your referring doctor

Insurance, Referrals
& Payment Info

New York Nephrology has contracts with many insurance carriers and will file participating insurance claims for your convenience. However, full payment is ultimately the patient’s responsibility.

We strongly recommend contacting your insurance carrier before your visit to determine whether your tests, procedures, and appointments are covered under your policy. This can help you avoid unexpected charges.

It is important to understand what services are covered by your insurance plan and where those services can be obtained. Some insurance companies may approve and pay for services as if they were in-network.

To determine your insurance benefits and eligibility, please contact your insurance carrier directly. Patients are responsible for payment for services that are not covered by their insurance plan.

Please bring your insurance card and photo identification to each visit. Let us know if there have been any changes to your personal, insurance, or contact information.

If your insurance requires a referral, please bring it to your visit. If you are having trouble obtaining a referral, a member of our staff will be happy to assist you before your appointment.

Co-pays and deductibles are required at the time of service.

Any remaining balance after submission to insurance is due within 30 days. While New York Nephrology will file participating insurance claims as a courtesy, patients are ultimately responsible for charges not covered by their insurance plan.

Chronic Kidney Disease Care

New York Nephrology is committed to early detection and complete care for patients with chronic kidney disease, also known as CKD.

Our providers work closely with nurses, nurse practitioners, and other care professionals to help ensure that kidney disease and related health concerns are monitored and managed appropriately. This type of coordinated care can help patients better understand their condition and support long-term kidney health.

Chronic kidney disease affects many adults and may be identified through blood tests, urine tests, imaging studies, or other medical evaluation. If kidney disease progresses, reduced kidney function can lead to additional health complications.
