INSURANCE AND FINANCIAL INFORMATION

We have contracts with many insurance carriers and will file participating insurance claims for your convenience. However, full payment is ultimately the patient’s responsibility. We strongly recommend contacting your insurance carrier prior to your visit to determine whether your tests and/or procedures will be covered under your policy so you do not incur any unexpected charges.

Benefits and Eligibilty
It is very important that you know what services are covered by your plan and where the services can be obtained. Some insurance companies will approve and pay as if they were in-network.

To determine your insurance benefits and eligibility, contact your insurance carrier directly. You are primarily responsible for payment for all services that are not covered by your insurance plan.

Required Documents
Please bring your insurance card and photo identification to each visit, and inform us if there have been any changes to your information.

If your insurance requires that you have a referral, please bring it to your visit. If you are having trouble obtaining a referral, a member of our staff will be happy to assist you prior to your visit.

Co-Pays and deductibles are required to be paid at the time of service. Any balances that remain after submission to your insurance will be due within 30 days.
